If we examine the world in the real sense, there are many diseases that only became prevalent in the fairly recent past. Most of these diseases are curable, but some are not. For many people, the group health insurance plan sponsored by their employer offers them the most affordable coverage. Group <strong>health insurance </strong>is exactly what it sounds like: a health insurance plan or plans offered to groups of people through their employers. Individual and family health insurance, on the other hand, is offered to individuals and families instead of employer groups, and it can be a much more attractive and affordable option than many people believe.<br />
Because<strong> individual and family health insurance </strong>is not offered through an employer, those who choose this type of insurance will pay the entire cost of the regular premiums. However, there is a wide range of plan types available, allowing smart consumers to maximize the coverage they are receiving for the money they&#8217;re investing in the plan. In some situations, they may even be able to save money compared to what they would have spent in premiums for an employer&#8217;s group health insurance plan. Many indemnity plans also require higher deductibles that must be met before the plan coverage will begin, and they also pay claims based on a percentage of the cost for the care. Because this network of providers has, in most cases, agreed to provide the treatment at a pre-set price, the care will cost less out-of-pocket for the consumer. care is covered with only a low percentage coinsurance or set co-payment amount required from the policy holder.<br />
There are three types of managed-care plans: HMOs, PPOs, and POS plans. These options are all based on provider networks and require their policy holders to pay for their health care depending on their tendency to seek care from in-network or out-of-network providers.<br />
In each category, there are dozens of available plans offering different levels and types of coverage that allow users to choose based on personal needs. Many plans require a deductible amount to be met for each plan year before coverage begins, and monthly premiums are likely to be lower for plans that have higher deductibles. This along with other factors affects how much the plan will cost the consumer to use. Therefore, a person who expects to seek health care only a few times a year will likely benefit by choosing a plan with a lower monthly premium. On the other hand, those who seek routine care and have a history of more physician visits, and/or who regularly fills expensive prescriptions, can best serve their medical needs with a plan requiring a higher monthly premium and low or no deductible.<br />
These are not the only factors that should be considered when choosing a plan. Someone who travels often may want to consider the possibility of needing to seek care while far from home and the advantages of an indemnity or a more flexible managed-care plan, so that unexpected out-of-network expenses can be covered. Women who expect to become pregnant during their plan year must carefully study the coverage offered to them during pregnancy and delivery. No plan is right for everyone; that&#8217;s part of the reason there are so many from which to choose.<br />
Making a smart choice requires thorough study of the plans available. The needs of every person who will be covered by the plan should be taken into account. With careful consideration and planning, those needs can all be met affordably through family and individual health insurance.<br />

			<content:encoded><![CDATA[<p>Recently, a bill was signed in Florida that would help establish the creation of a<strong> low cost health insurance plan</strong> for Florida residents.<br />
Unfortunately, amid all the fanfare, very few people stopped to take a close look at what was actually being proposed. Permit me to enlighten you on some of the more important aspects of the proposed plan.<br />
First of all, it is not a mandated <strong>health insurance plan</strong>. In other words, an insurance carrier would have to decide if they even want to create a plan to sell. This is NOT A PLAN THAT THE STATE OF FLORIDA IS PROVIDING. Please understand that Florida is in the midst of an extreme budget crunch and is not spending a single dime on this plan. Governor Crist has made that very clear. The cooperation of the <strong>health insurance</strong> industry will be required to make this work. As of now, there are no plans available.<br />
Second, this is not a major medical plan. If you have a medical crisis and this is your only health insurance,<strong> </strong>you are in a lot of trouble. It is a minimal plan with a few doctor visits, a physical, perhaps some generic prescription coverage (the drugs you already get in Wal-Mart for $4) and a few other benefits. Surely, none of you ever thought that you would get a complete major medical plan for $150 a month.<br />
Next, the plan is for someone who has had no health insurance for six months. This means that in their infinite wisdom, Florida&#8217;s representatives decided that if you lose your job and cannot afford your COBRA insurance, they want you to be without any <strong>health insurance</strong> at all for at least six months. We have a real brain trust in Tallahassee don&#8217;t we.<br />
Now, you are really going to love this. The plans are going to sell for about $150 (projected price but who knows) and give you the most minimal of benefits. But that is the cost for an individual. A family will be much more. As a matter of fact, if you have a healthy family, a traditional major medical plan with real benefits might be less expensive.<br />
For example, at FLQUOTE a Florida Health Insurance website a 40 year old male in Pasco County can get an excellent PPO plan for about $130 a month. We are talking about a complete multi-million dollar, major medical plan. Not a minimal plan.<br />
Who will this plan help?<br />
I am still trying to figure that out. This is supposed to help those of you who cannot get health insurance. But, it will not pay for your expensive brand name prescriptions; it will not cover extensive hospitalization or expensive testing procedures. For those of you who think that dental coverage is part of health insurance &#8211; it is not. It will not cover any dental or vision costs.<br />
If you are poor, you are much better off on Medicaid. If you are medically qualified, you will get far more coverage for your money with a traditional health insurance plan.<br />
Have politicians created a created a, half-way measure to pander to the voters? Is it &#8220;better than nothing&#8221;? If you are paying $1,800 a year for a few office visits, you have to wonder if it is even worth it.<br />

Insurance companies are using these questionnaires as well as an exclusion period in order to defend themselves from people with pre-existing conditions that are seeking medical<strong> insurance.</strong><br />
In the state of Illinois people that are applying for an individual health insurance plan can be turned down at the insurance company&#8217;s discretion due to pre-existing conditions unless that person is eligible for an Illinois HIPAA health insurance plan.<br />
In the state of Illinois they follow HIPAA laws very strict. The Health Insurance Portability and Accountability Act created in 1996 and effective in 1997 provides protection for people that have medical pre-existing illnesses. The law protects people by limiting their exclusion period when purchasing health insurance, lowering the chances for a member with a pre-existing condition to lose coverage, providing protections when they change jobs and guaranteeing that your health insurance policy gets renewed at the end of your coverage year.<br />
The law however, has not eliminated the ability of individual carriers of denying health insurance to pre-existing condition people or exclude medical conditions. The only guarantee issue provisions lie in State sponsored plans and insurance company funded plans. What HIPAA does provide is for guaranteed acceptance health insurance coverage for people that meet 6 HIPAA requirements. When someone meets these 6 requirements they are considered &#8220;HIPAA eligible&#8221; and can qualify for a guaranteed issue HIPAA health insurance plan. The 6 requirements for HIPAA eligibility can often be the only avenue of <strong>health insurance</strong> coverage available to some high risk individuals with major pre-existing health conditions.<br />
Some of the most important insurance companies in the state of Illinois handle pre-existing conditions a little bit differently, because of this it is important to do some research and actually shop around for a policy before deciding to apply. Individual plans have more exclusion that group plans and that is why they are quite a bit less expensive, because they are more restrictive.<br />
Aetna <strong>Health Insurance</strong> who is one of the &#8220;big dogs&#8221; in the health insurance business across the United States is a primary example of exclusion period. They offer a 365 day period starting from the day of enrollment, in which a person with a pre-existing condition is not covered. It is important to note however, that if the person that has a pre-existing condition has had prior creditable coverage within 63 days immediately before the signature of the application; then the exclusion period will be waived.<br />
Another example of this can be seen with Blue Cross and Blue Shield of Illinois, who is one of the 39 independent, community-based insurance companies that make up the national Blue Cross Blue Shield network. Since they are independent that means they might not have the same provisions as Blue Cross Blue Shield companies in other states. In Illinois, BCBS requires a member with a pre-existing condition to wait a 365 day exclusion period from the day that they sign the policy before receiving coverage for their illness.<br />
Compared to individual coverage, group plans are a little better. They cannot turn you down due to a pre-existing condition, which makes group plans more expensive. Under HIPAA law an employer can only deny pre-existing condition coverage if the person is diagnosed, receives treatment or has care and treatment 6 months before the enrollment date. A good thing to note is that pregnancy cannot be accounted as a pre-existing condition by an employer insurer.<br />
The total time a person can be excluded from a group health plan if they have a pre-existing condition is 12 months after enrollment (18 months if they enroll late), for this reason it is important for a person to sign up for health insurance as soon as they are offered it (if not you can be subject to 18 months instead of 12). Fortunately for some, the time can be less in case that they were covered by an<strong> insurance</strong> company for the 63 days before enrollment. Also, an insurer cannot deny coverage to a small employer (2-50) under HIPAA law.<br />
Finding Illinois health insurance coverage when one has a pre-existing condition can be very tough. Not to mention that pre-existing conditions cover everything from cancer, HIV, Hepatitis C and even high cholesterol. It is key however, for a person that has a pre-existing condition to know all the exclusions and their rights that are provided under the HIPAA law. This is important because once you know your rights, you will be able to be more knowledgeable about the subject and avoid long exclusion periods.<br />
You will only find good health insurance that covers pre-existing conditions if you shop around.
